چکیده

A 12 years-old boy was admitted in coma without motor deficit, with normal CT scan, CSF with lymphocytosis, and EEG showing B(I)RDS (Figure A). Continuous EEG (24 hours) and subsequent serial intermittent EEGs were performed. Seizures were not documented. Patient developed orofacial diskynesia and coreoathetosis movements affecting superior limbs. Later, EEG showed asymmetric EDB pattern (Figure B). MRi was unremarkable. After immunotherapy, his level of consciousness improved. Anti-NMDAr antibodies were detected in serum and CSF. B(I)RDS were described in critically ill patients, often associated with brain injury and seizures, but no reported in Anti-NMDAr encephalitis1. EDB has been described in AntiNMDAr encephalitis; however, their clinical significance remains to be determined2.

EEG extreme delta brush; could be an ictal pattern in patients with anti-NMDA receptor encephalitis

INTRODUCTION: Anti-NMDA receptor (NMDAr) encephalitis associated syndrome includes neuropsychiatric symptoms, impaired consciousness, seizures, autonomic instability, and hypoventilation. The electroencephalographic (EEG) activity throughout the course of the disease has not been documented enough. We reviewed electroclinical data of patients with NMDAr encephalitis contributing to characterize...

Extreme delta brush evolving into status epilepticus in a patient with anti-NMDA encephalitis

Extreme delta brush (EDB) is an EEG pattern unique to anti-NMDA encephalitis. It is correlated with seizures and status epilepticus in patients who have a prolonged course of illness. The etiology of the underlying association between EDB and seizures is not understood. We present a patient with anti-NMDA encephalitis who developed status epilepticus evolving from the high frequency activity of...

Early and persistent ‘extreme delta brush’ in a patient with anti-NMDA receptor encephalitis☆

Since its original description in 2007, anti-N-methyl-d-aspartate receptor (anti-NMDAR) encephalitis associated with an ovarian teratoma is an increasingly recognized etiology of previously unexplained encephalopathy and encephalitis. Extreme delta brush (EDB) is a novel electroencephalogram (EEG) finding seen in many patients with anti-NMDAR encephalitis. The presence of this pattern is associ...

Brief potentially ictal rhythmic discharges in critically ill adults.

IMPORTANCE Brief potentially ictal rhythmic discharges, termed B(I)RDs, have been described mainly in neonates, and their significance in adults remains unclear. OBJECTIVE To describe the incidence of B(I)RDs in critically ill patients and investigate their association with seizures and outcome. DESIGN, SETTING, AND PARTICIPANTS We reviewed the records of prospectively identified patients w...

B(I)RD Watching: A Way to Stratify Seizure Risk?

Commentary Seizures are deceptively difficult to define. Clinical seizures with an EEG correlate are rarely problematic, but seizures can be purely clinical, with no scalp EEG correlate, or conversely, purely electrographic, with no apparent clinical correlate. The latter is a common occurrence in neurologically critical patients. There is recognition that seizure definitions set somewhat arbit...
